Primary Care

Our primary care team consisting of physicians, nurse practitioners, physician assistant, registered nurses and knowledgable staff are here to care for our patients to help them achieve their optimal health.

Social Work Services

Our Registered Social Worker offers individual counseling to our patients. Self-referrals are accepted from rostered and unattached patients in our community.

Nutrition Counseling

Our Registered Dietitian offers individual counseling to patients with chronic disease that would benefit from nutritional guidance.

Introduction to Starting Solids

Is your baby between 4 and 6 months old? Are you wondering if they’re ready to eat, and what foods you should start with? Do you have questions about safe feeding, allergies or preparing baby food? Join our Registered Dietitian for an introduction to starting solids. We’ll cover when to start feeding your baby, what to feed them, how to safely prepare baby food, developing healthy mealtime habits and more!

The workshop will be held online, so you can join with your little one from the comfort of your own home.

Our Nurse Practitioners offer womens’ health screening, prenatal care and more.

Learn ways to manage your mood and improve your mental health by understanding the connection between the way you think, what you do and how you feel..

Diabetes Education

Our Registered Dietitian has partnered with SFMH for the Diabetes Education Program to assist patients with the tools to manage this chronic disease. Sign up today for our monthly education class.

Hypertension Program

Our hypertension program is for patients with high blood pressure. Our RPN is the program facilitator. This includes a one-week BP monitor lending program for patients requiring short term self-monitoring of their blood pressure at home. Call 613-756-9888 ext 5 for more information.

Smoking Cessation Program
